Let The following be a Digital Waiver and Video Release to participate in Virtual Lessons.

Couples must each Digitally Sign their own copy of the form for submission. Thank-You.

By signing the form Digitally, I the signee, agree to waive and release Arthur Murray Dance Centers of Woodbridge all liability that may be incurred in connection with my participation in virtual dance lessons. I agree to assume all responsibilities, damages and costs incurred as a result of an injury sustained from participating in virtual interactive lessons at home. I agree not to hold Arthur Murray Dance Centers and it’s staff or instructors responsible in any way for such injury. Further The online lesson will be recorded for the usage by the Student, Instructor and Studio for the process of administering the enrolled dance program.
